General discussion • Raspberry Pi 5 Boot Failure: "Failed to Open SSD" Error

Hi everyone,

I’m experiencing inconsistent boot issues with my Raspberry Pi 5. Here's a detailed summary:

Setup Details:
Power Supplies Used:
- Official Raspberry Pi 5V/3A charger
- Phone charger (25W) via USB-C
SD Cards
- Tried two SD cards, both freshly flashed with Raspberry Pi OS 64-bit using Raspberry Pi Imager.
- Also tested booting with the SD card via a USB reader.
Voltage Measurement:
- Across GPIO 5V pin and GND:5.46V

Issue Description:
1. Without an SD card:
- The Pi sometimes boots successfully, but not consistently.
2. With an SD card:
- It attempts to boot, flashes the green LED briefly, but fails with a **"Failed to open SSD"** error.
- The red LED stays solid after failure.
3. Using a USB SD card reader:
- The same boot issue persists (no improvement).

Troubleshooting Tried:
- Reflashed multiple the 2 SD cards with Raspberry Pi OS 64-bit using Raspberry Pi Imager.
- Removed peripherals to ensure minimal power draw.
- Tested with two different power supplies (official and phone charger).
- Inspected the SD card slot and GPIO pins for physical damage (none found).

Observations:
- Bootloader seems functional as it occasionally boots without the SD card.
- Boot failure is more frequent with the SD card inserted.
- USB SD card reader does not resolve the issue.

Questions:
- Could this be a hardware issue with the SD card slot or power supply circuit?
- Are there additional steps to reflash or debug the bootloader?
- Any insights or suggestions to resolve this would be greatly appreciated.
