If you just bought the Pi4 (new out of the box) you should see boot diagnostics screen.I just bought a Raspberry PI 4 used and I have not gotten an SD card setup to use but hooked it up to a monitor and it displays nothing .
I will set up a proper micro SD card but still thought it would display something on the monitor because the fan comes on and red led comes on and the green one flashes .
So does it need the SD card before it displays anything ?

If you've bought the Pi4 second handed and DON'T see boot diagnostics it's a pretty old one that needs an EEPROM update urgently (but you will need a uSD for doing so). OS will take care of updating this automatically.
If you don't see anything with the Pi4 connected to your screen and powered up without a uSD card another common problem is the HDMI interface cable.
Are you using an microHDMI to HDMI cable or some dodgy adapter in between?
